HOW TO PREPARE FOR YOUR VISIT
Welcome to Waco Cardiology Associates. In order to help make your visit a pleasant experience, we are going to ask you to supply a few things to us in advance.
Most often you will be speaking with an appointment scheduler who you will be verbally communicating most of the required information such as your personal demographic information, your primary care physician, insurance information and reason for your appointment.
We will be happy to retrieve any document from your past medical history such as Echocardiogram, EKG, Cardiac Catheterization that will help your Cardiologist better assess your condition or concern at your visit, whether the records are from a past hospital stay, an emergency visit, or records from your Primary Care Physician. This will require you to sign a Release of Medical Records form. Please complete the form and send to the office of your scheduled appointment.
If your medical insurance requires a referral, you will need to get in touch with your Primary Care Physician to let them know when your scheduled appointment is and ask them to enter a referral for you.

Please arrive 15 minutes prior to your New Patient appointment.

Medical Records
The following forms authorize Waco Cardiology Associates to receive your medical records, including but not limited to EKG, Stress Test, Cardiac Catheterization, Labs, Carotid Ultrasound, Echocardiogram, chest x-ray.
