How to Prepare for Your Visit

Welcome to Waco Cardiology Associates. In order to help make your visit a pleasant experience, we are going to ask you to supply a few things to us in advance.

Our scheduling staff may contact you to obtain your demographic information, insurance information and reason for your appointment.

We will obtain copies of your medical records from your referring provider / primary care provider.

If your medical insurance requires a referral, you may need to contact your Primary Care Physician to let them know when your scheduled appointment is and ask them to enter a referral for you.

We invite you to make use of the following forms (PDF) before your appointment.

Please arrive 15 minutes prior to your New Patient appointment.

Health Insurance Information

We are participating providers with Medicare and most Medicaid and commercial insurance plans.

What if your plan isn’t on our list?

We recommend that all patients check with their benefits representative at their health plan prior to seeking care. We will always do our best to maximize the health plan benefits you have.

Medical Records

The following forms authorize Waco Cardiology Associates to receive your medical records, including but not limited to EKG, Stress Test, Cardiac Catheterization, Labs, Carotid Ultrasound, Echocardiogram, chest x-ray.

Please download, fill out and present this form to your cardiologist or hospital.
